Flow 14 — Supplier corrective action and effectiveness
Turn one contained supplier NCR into an independently issued SCAR, retain immutable supplier-response revisions, verify systemic effectiveness and close with attributable evidence.
Overview
SCAR drafting, issue, supplier-response capture, decision, effectiveness and closure are zero-stock Quality facts. They never hold, release, move or adjust inventory and never approve, suspend or otherwise rewrite the supplier master.
Quality Inspector → Supplier Quality Engineer → different QA Head → Supplier Quality Engineer → QA Head → Buyer / Procurement Manager / Auditor read-only
Process
- 1Contained supplier NCR
Start from one exact supplier failure with proven containment.
Quality fact · Inventory unchanged - 2SCAR draft
Supplier Quality defines the problem, requested response and due date.
Request package · Inventory unchanged - 3Independent issue
QA verifies and records the manual delivery reference.
Issued Quality request · no supplier-status change - 4Versioned response
Capture supplier root cause and implemented systemic action.
Response evidence · Inventory unchanged - 5Independent decision
QA accepts or rejects; rejection preserves history and reopens response work.
Decision fact · Inventory unchanged - 6Effectiveness
QA tests sustained results against the accepted response.
Verification gate · Inventory unchanged - 7Closed
Retain closure acknowledgement while the parent NCR stays independently governed.
Closed SCAR · supplier/NCR unchanged
Role handoff
Quality Inspector → Supplier Quality Engineer → different QA Head → Supplier Quality Engineer → QA Head → Buyer / Procurement Manager / Auditor read-only

Detailed steps
- 1
As Quality Inspector, raise a SUPPLIER NCR with the registered vendor and record CLEAN evidence-backed containment. Confirm an OPEN or non-supplier NCR cannot start a SCAR.
- 2
As Supplier Quality Engineer, open Quality → Supplier corrective action, select the contained NCR and create the issue package with exact problem statement, requested containment/root cause/corrective action, response due date, supplier routing reference, classification and CLEAN evidence.
- 3
Submit the draft. Confirm Supplier Quality cannot issue it, then sign in as a different QA Head and independently record issue date, delivery method, delivery reference and decision note.
- 4
Return as Supplier Quality Engineer and capture the supplier representative, response reference, received/implementation dates, containment, structured root cause, contributing factors, correction, corrective/preventive action and CLEAN evidence.
- 5
Confirm the response capturer cannot decide their work. As QA Head, reject an incomplete response and verify it remains immutable as version 1, then accept a complete separately captured version 2.
- 6
Confirm Supplier Quality cannot verify their captured action. As QA Head, record an independent sustained-result method, observation, residual risk and CLEAN evidence; an ineffective result returns the SCAR to ISSUED for a new response version.
- 7
Close only after the latest review for the current accepted response is EFFECTIVE and a supplier closure/acknowledgement reference plus CLEAN evidence exists. Confirm the linked parent NCR does not close automatically.
- 8
As Buyer, Procurement Manager and Auditor, read the full chain and confirm every attempted mutation is denied, supplier AVL/registration state is unchanged and current stock plus stock ledger are unchanged.
Field-by-field practice data
Issue a supplier corrective-action request from one contained supplier NCR. The page shows sample set A; the workbook includes independent A, B and C values.
Field names match the current application labels. Select existing approved records where shown, leave optional values blank unless they genuinely apply, and never copy fictitious TRAIN values into live work.
Author the SCAR
Select the contained supplier NCR and define the response package. A different QA Head issues it.
| Exact field name | Requirement | Sample value A | How to use it |
|---|---|---|---|
| Contained supplier NCR | Select existing | TRAIN-A-NCR-001 | Select the exact contained source NCR. |
| SCAR title | Required | TRAIN-A thread-control corrective action | Use a supplier-facing factual title. |
| Supplier contact / routing reference | Required | TRAIN-A-SUPPLIER / Quality contact | Use the approved contact route. |
| Response due date | Required | 2026-09-30 | Set the controlled supplier deadline. |
| Classification | Required | RESTRICTED | External handling must remain within policy. |
| Problem statement | Required | Incoming batch failed approved thread-dimension criteria. | Copy the approved problem boundary without adding unsupported claims. |
| Requested containment | Required | Identify affected stock and stop further shipment pending review. | State immediate supplier action expected. |
| Requested root-cause response | Required | Provide method, verified cause and escape point. | Define the required analysis. |
| Requested corrective action | Required | Provide implemented systemic correction and effectiveness plan. | Define closure evidence expected. |
Expected result: The supplier response lifecycle is immutable and does not change supplier approval or stock by itself.
End state
One supplier action retains contained-NCR provenance, independent issue, rejected and accepted response versions, effectiveness and closure evidence while procurement, supplier approval, parent-NCR closure and inventory remain separately governed.
